How do I apply for payment processing?

Fill out one of the forms below to sign up for payment processing through TogetherPay, CircuiTree’s integrated payment processing platform.

Controlling Officer

The forms need to be signed by a controlling officer at your organization. A controlling officer is defined as:

  • An owner, employee or volunteer authorized to conduct business and enter into contracts on behalf of the merchant. For non-profit organizations, you will be required to list a volunteer or administrator with significant decision-making power.

Verification

Two pieces of information that we need to setup your processor is your organization’s Legal Name and Employer Identification Number. This information is used to verify your organization through an automated process.

In the rare case we are not able to verify your organization through the automated process, we will reach out to the controlling officer to gather more personal information to verify them

  • Full Name
  • Email
  • Phone Number
  • Last 4 digits of Social Security Number
  • Date of Birth
  • Current Residential Address (No PO Box)
  • Prior Residential Address (No PO Box) – if moved within the past two years
  • Identity Verification
    • Driver’s License
    • Military ID
    • State ID
    • Passport
  • Address Verification – must match current residential address
    • Voided Check
    • Utility Bill
    • Bank Statement

Steps to Complete Forms

The above forms can be signed electronically using DocHub. Follow these steps to sign the form if you don’t currently have a DocHub account. Once
